Working as part of a multidisciplinary team, you’ll deliver personalised, evidence-based care through our award-winning Plena Care Pathway, recognised with the Innovative Product Excellence Award at the 2023 Allied Health Awards. You’ll conduct assessments, develop tailored treatment plans, deliver interventions, prescribe mobility aids, and collaborate closely with residents, families, nurses, GPs, Allied Health Assistants, and other healthcare professionals to achieve meaningful outcomes.

With opportunities to work across private homes, retirement villages, and residential aged care facilities, no two days are the same. As our community aged care services continue to grow, you’ll also support clients across a range of funding streams, including Support at Home, STRC, CHSP, and private referrals, creating a diverse and rewarding caseload.
