The Plena Care Pathway was the winner of the 2023 Allied Health Award for Innovative Product Excellence and is Plena Healthcare’s industry-leading physiotherapy & occupational therapy service model.

The Plena Care Pathway supports residents with access to tailored allied health services, through their entire residential aged care journey. The Pathway is available to all residents; it incorporates mobility and functional assessments and co-designed programs, falls management frameworks, care plan reviews, one-to-one therapy interventions and dynamic group-based active ageing services.

Plena developed the Plena Care Pathway for over 18 months before it went live with AN-ACC in October 2022. The program is now live in over 450 aged care homes nationally as the immediate clinical and commercial allied health solution for our providers and residents.

Throughout a recent Plena Care Pathway implementation DeMorton Mobility Index (DEMMI) improved in 88% of participants, 85% saw pain scores improve, and 90% achieved their therapy goals. Nursing & care staff reported benefits including reductions in behaviours, falls and pressure injuries and improvements in appetite, sleep hygiene and mood/anxiety.

 

Importantly, in a sample of 1611 residents, 87% of respondents reported a positive user satisfaction score indicating the Plena Care Pathway impacted positively on their health & wellbeing.

Active Ageing uses the SUNBEAM Falls Prevention Program at it’s core – an evidence-based program known to reduce falls by 55% in the residential aged care population.

We seamlessly integrate the pathway with additional user pay services if the resident chooses to opt-in, and our customers have helped us promote the pathway to prospective residents as a genuine, revolutionary model of effective care.

 

We are passionate advocates for sustainable care models & positive outcomes for clients in residential aged care. We are currently involved in the Expansion of Quality Indicators pilot study to establish benchmarks for essential services such as Allied Health. The study will influence the expansion of National Quality Indicators Program to include allied health services, aligned with the Aged Care Reform Road Map to 2025.
