Ready to make a referral?

Accessing Plena Healthcare’s allied health services in your home is as simple as completing our referral form. We accept referrals for Home Care consumers and NDIS participants, as well as privately funded clients. Individuals with private health insurance may be eligible for a rebate from your insurer.

After sending us your completed referral form, please allow 48 hours for our team to assess your allied healthcare needs and allocate the appropriate therapist. Our team will call to arrange an appointment time and conduct risk assessments, then send an email confirmation. 

Appointment reminders are sent via SMS 48 hours and 24 hours before the scheduled appointment time. 

Following the initial assessment with our therapist, a recommendation report will be sent for approval of follow-up cycle treatment.  

Our multi-disciplinary approach
