with Let’s Get Care and Plena Healthcare

Staying active, independent, and safe at home starts with feeling confident about your wellbeing.

That’s why Let’s Get Care has partnered with Plena Healthcare to introduce Plena’s new  Get Moving program.

‘Get Moving’ is designed to help you take a proactive approach to your health, mobility and everyday independence. It supports your physical wellbeing, movement, and confidence – so you can continue living well and doing the things that matter most to you.

 

What is ‘Get Moving’?

 

If you’ve noticed aches, stiffness or daily tasks becoming more challenging, ‘Get Moving’ could help you better understand your mobility needs, and identify proactive steps you can take to support your independence and help you live well at home.

‘Get Moving’ is designed to support a proactive approach to your mobility, wellbeing and everyday independence. It is an early‑intervention service that helps identify your mobility needs before they become bigger challenges. It works alongside your existing care plan, taking your current goals and circumstances into account to create a personalised program focused on helping you stay active and confident at home.

‘Get Moving’ is designed to focus on:

  • Strength, balance, and mobility
  • Building confidence and independence at home
  • Active and healthy ageing

 

Get started with ‘Get Moving’ in three simple steps:

 

Step 1: A mobility assessment
Your Mobility Coach, an experienced Plena Healthcare professional, will take time to understand you and your current mobility and wellbeing through a 30mins phone conversation.

Step 2: Goal Setting
Together with your mobility coach, you will set meaningful goals to help you stay active and independent.

Step 3: Recommendations
Following the assessment, you will receive a report with recommendations around your ongoing care that are specifically tailored to your needs, abilities and goals.

If you would like to consider additional support, Plena Healthcare’s allied health team can provide ongoing services in the following areas:

  • Physiotherapy
  • Occupational therapy
  • Dietetics
  • Speech pathology
  • Podiatry

 

What does it cost?

The Get Moving Program

is charged at the same hourly rate as a standard Allied Health consultation. If you have available funds in your Support at Home package, you can use these to cover the cost. In this case, the service charge to your package will be $209.

If you prefer to pay privately, the program is available at a discounted rate of $196 per hour.

Any ongoing Allied Health services you choose to continue with after the program will be charged at the standard rate of the provider delivering those services.

 

What this means for you

 

‘Get Moving’ is an additional optional service, delivered by Plena Healthcare. If you choose not to take up this opportunity, your care plan will remain unchanged and there will be no interruptions to the care and support you receive.

While we are excited to provide you with this opportunity to access the Get Moving Program, you are not obliged to use Plena for allied health care services you access as part of your package and you are free to exercise choice in your selection of provider.

Our Dietitians are experienced at creating personalised nutrition plans to address your specific dietary needs. We provide recommendations that combine best practice nutrition advice, individual preferences and experiences. Our speech pathologists specialise in assessment, treatment and management of communication and swallowing difficulties. More specifically, our team has extensive experience in treating speech difficulties (dyspraxia or dysarthria), swallowing difficulties (dysphagia), language or communication difficulties (aphasia), and voice difficulties (dysphonia).
