Plena Healthcare offers a variety of services direct to your doorstep, so you can lead the life you want and deserve in your very own home. We support the elder community and believe in providing the best quality care so people can live healthier, happier and more independent lives.

With mobile teams across the country, we’re able to support the elderly, and those requiring in-home rehabilitation with the necessary healthcare services from the comfort of their own homes. If face-to-face visits aren’t an option, we also offer telehealth programs.

We aim for regular visits from the same team member to facilitate connection and personal understanding, and we pride ourselves on providing a client-centred approach, with an emphasis on ongoing communication between all people involved in the client’s care.

Physiotherapy

Our physiotherapists are highly experienced in managing a wide range of acute and chronic conditions which affect movement, function, and independence. Our services are delivered through a patient-centred lens that ensures our clients have the best possible opportunity to achieve their goals.

Occupational Therapy

Our occupational therapists will work with you to first determine what is important to you, and how you would like to live your life. From there, we identify any challenges that may be preventing you from enjoying your desired lifestyle. This includes a review of self-care, environmental assessments and physical rehabilitation to enable participation in the activities of everyday life.

Podiatry

Every podiatry client we work with has a unique personality, circumstance, and unique care needs. During our consultation we provide a thorough explanation of our services, and offer you guidance and assistance in choosing what option is best for you or your loved one.

Dietetics

Our dieticians are experts at creating personalised nutrition programs to address your dietary needs. We provide recommendations that combine best practice nutrition, with a practical and quality of life-based understanding with a focus on the client and their experience. Our team can also provide advice and recommendations to support specific weight management strategies and goals.

Speech Pathology

Our experienced speech pathologists are here to support your swallowing, speech, voice, language and/or communication needs. When conducting swallowing assessments, we observe how you normally eat and drink, provide practical strategies, swallowing exercises and advice on how to prepare food and fluid modifications to ensure you are eating and drinking safely. We listen to your concerns, ask questions, conduct tests and make clinical observations and recommendations tailored to your individual needs.
